Please be sure the form and questionnaire is completed as soon as possible to secure your spot!

Please be advised we will be checking your temperature at entrance screening.

If you or a member of your household is or has been feeling ill with a cold, flu-like symptoms or a fever of 100 degrees or higher, we strongly recommend for you to please stay home and worship with us via our livestream on Facebook or YouTube.

COVID-19 QUESTIONNAIRE

1. Have you or anyone in your household had any of the following symptoms within the last 14 days: sore throat, cough, chills, body aches, shortness of breath, loss of smell, loss of taste, fever at or greater than 100 degrees Fahrenheit?

2. Have you or anyone in your household tested positive for COVID-19?

If YES, have you quarantined for 14 days? If not, how many days have you been quarantined?

3. Have you or anyone in your household traveled out of Monterey County within the last 14 days?

4. To the best of your knowledge have you or anyone in your household been in close proximity to any individual who tested positive for COVID-19?

If YES, have you been tested and/or waiting for test results?

5. Have you or anyone in your household attended a large gathering within the last 14 days?

A birthday party, wedding, large family gathering, etc.
